James Lind’s 1747 scurvy experiment aboard HMS Salisbury is often described as one of the earliest controlled clinical experiments in modern medical history. It was important, but calling it simply “the first clinical trial in history” hides a much longer history of comparative medical testing.

What Lind actually did
Lind selected 12 sailors with scurvy, kept their basic conditions and diet broadly similar and divided them into pairs receiving different contemporary treatments. The pair given oranges and lemons improved markedly compared with the others.
By modern standards the experiment was tiny, unblinded and not randomised. Its importance lies in the deliberate comparison of several treatments under more controlled conditions than was common in routine practice.
What it did not prove
Lind did not discover vitamin C, and his work did not immediately produce a universal naval policy. The biological explanation for scurvy came much later. Nor did modern randomised controlled trials emerge directly from this single experiment; clinical research methods developed through many later advances in statistics, trial design, ethics and regulation.
The historical value of Lind’s experiment is therefore methodological rather than mythical: it illustrates the growing importance of comparing outcomes instead of relying only on theory and authority.
