Placebos became important to medicine not because inactive treatments mysteriously cure a fixed percentage of patients, but because researchers needed better ways to distinguish treatment effects from expectation, natural recovery, reporting effects and other changes that occur during a study.

History of placebo research

Henry Beecher’s 1955 paper The Powerful Placebo was highly influential and popularised the idea that roughly a third of patients might respond to placebo. Later methodological work challenged that interpretation because improvement observed in a placebo group can also reflect spontaneous recovery, regression to the mean and measurement effects.

A true placebo effect is better examined by comparing placebo treatment with a no-treatment or otherwise appropriate control condition. Placebo-controlled trials, meanwhile, are primarily designed to estimate whether an intervention performs better than an inactive comparator under specified conditions.

Expectations, conditioning and the clinical encounter can influence some subjective outcomes, particularly symptoms such as pain or nausea. That does not mean placebo responses can substitute for treatments that alter the underlying course of serious disease.

The history of placebo research is therefore also a history of improving trial design and learning to separate multiple sources of change that can look similar in clinical data.
