Vaccination schedules are designed to recommend particular vaccines at ages and intervals when they are expected to provide useful protection. In the United States, those schedules are shaped by scientific evidence, regulatory decisions, recommendations from the Advisory Committee on Immunization Practices (ACIP) and formal adoption by public-health authorities. In 2026, court proceedings also affected which recent policy changes could take effect.

Which US Vaccine Schedules Are Currently Published?

The CDC states that a preliminary federal court order issued on 16 March 2026 stayed ACIP votes taken during its June, September and December 2025 meetings. The order also stayed a 5 January 2026 decision memorandum that had revised the childhood immunisation schedule.

As a result, the CDC identifies its 2 July 2025 childhood and adult schedules as the current schedules. The agency notes an April 2026 amendment relating to an earlier recommendation on RSV vaccination for certain higher-risk adults aged 50 to 59. Because litigation and policy can develop, clinicians and patients should consult the current CDC pages rather than rely on a saved copy or an older news report.

How ACIP Recommendations Are Developed

ACIP is a federal advisory committee made up of medical and public-health experts. It develops recommendations on the use of vaccines in the US civilian population. The CDC explains that ACIP uses an explicit evidence-based process built around the GRADE method and an Evidence to Recommendations framework.

The process considers more than whether a vaccine produces an immune response. Reviewers assess the balance of benefits and harms, the certainty of the evidence, the values and preferences of affected groups and relevant health-economic information. Published evidence tables and recommendation frameworks are intended to make the reasoning easier to examine.

Why Timing and Dose Intervals Matter

A vaccination schedule is not simply a list of products. The recommended timing reflects factors such as the age at which a person is vulnerable, how the immune system responds, how long protection is expected to last and whether additional doses are needed to build or restore protection. The World Health Organization notes that infants can be especially vulnerable as protection transferred during pregnancy and breastfeeding declines during the first months of life.

Changing the timing, number or target group for a vaccine can therefore affect both individual protection and the operation of public-health programmes. Proposed changes should be assessed using transparent evidence and clearly communicated guidance, especially when recommendations are being discussed in political or legal settings.

Policy Debate Is Not the Same as Clinical Advice

Public debate may focus on the membership of advisory bodies, government priorities or court challenges. Those issues can influence how policy is made, but they do not provide personalised medical advice. A person’s recommended vaccines may depend on age, pregnancy, occupation, travel, previous doses, health conditions, medicines and other clinical factors.

Patients should use the current official schedule as a starting point and discuss individual decisions with a qualified healthcare professional. Urgent health decisions should not be based solely on political commentary, social-media posts or a general news article.
